  • INSTRUCTIONS FOR STAKEHOLDERS

    1. You are required to provide feedback specific to the relevant team member(s) for both the mid year and end of year periods. Your feedback will neither be shared verbatim nor attributed to you. Instead, it will be consolidated with management’s feedback.
    2. After submitting your mid year feedback, you will receive a copy sent to the email address you provide in the previous field (check your spam/junk folder if you do not see the email in your inbox). This email will include an edit link, which you should use to access the same form to submit your end of year feedback. Please ensure you retain this email.
    3. The feedback cells below are not user-friendly (e.g., text wrapping and line breaks are not supported). Due to its limitations, and to keep the interface concise, you are advised to draft and finalise your feedback in another source before copying and pasting it into the relevant cell.
