  • CDHN Individual Membership Form

  • This form is for people who want to join CDHN as individual members. You can apply through your job or volunteer role in any sector—private, public, community, voluntary or social enterprise or simply as someone who supports what we do. To be an individual member you must:

    • Live in Northern Ireland, Ireland, England, Scotland, Wales, the Isle of Man, or the Channel Islands.  If you live elsewhere, you can still join us as a Supporter 
    • Support CDHN Values, Mission and Vision

    As an individual member, you’ll have voting rights at the Annual General Meeting (AGM). If you’d prefer not to do that but still want to be involved, you can join as a Supporter instead.

Our members come from a wide range of backgrounds—small community groups, large charities, health and social care (including GPs, pharmacists, and social workers), education, and government. Understanding this helps us show our reach and tailor our communications to your needs.
