This application form is intended for individuals to submit a youth golf program for instruction and/or any golf access and playing opportunites (club membership fees; tournament/event fees, etc) for approval by the NENY PGA REACH Foundation for financial funding support of said program(s).
There are 2 categories you can apply for:
Learn to Play (Instruction/Clinics/Ext.) OR
Play to Learn (Greens Fees, Annual Memberships/Ext.)
Your responses will help our Foundation evaluate the need for assistance and utlimately approve and promote your preferred programming. Please note that submission of this form does not guarantee approval or funding, as Foundation & Committee will review approvals and funding will be based on eligibility, available resources, and program guidelines. All information provided will be kept confidential and used solely for the purpose of reviewing this request.
