These retreats are hosted as a partnership of ECD youth network churches, and are not specifically ECD events; the ECD supports the work of youth ministries across the district and shared youth network events
Instructions for using this form

This form is required for the registration of your youth for the upcoming ECD Youth Alliance Winter Camp (Eastern Canadian District). It will provide us with important personal information, consent for transportation, and confirmation of payment. Please complete it carefully and submit it before the deadline to secure your spot.
Activity: 2026 Jr. High (Grades 6-8) Winter Camp - ECD Youth Alliance

Date of Activity: January 30 – February 01, 2026

Location: 39 Joy Bible Camp Ln A, Bancroft, ON K0L 1C0

Mode of Transportation: School Bus

Activities upon arrival: A Weekend to retreat with God & Friends

Sleeping Arrangement: 10:00pm - 6:00am

Ratios of staff: Youth Pastor and 3 Leaders (4)

Students will be participating in: Dynamic workshops, Bible studies, Worship time, Sports (Basketball, Soccer, Volleyball) playing cards, some board games, mini putt, gym activities, Tubing, campfire and more!.

Registration Rates:

$210 per person (until January 10)
+$25 Late Fee after Januray 10

Payment:

We prefer cash payment as a priority, if possible.

This will help us to properly identify that your payment is for the retreat.

I hereby consent to the participation of my/our child in this supervised activity.While every precaution is taken for the safety and good health, some sports and activities carry with them the inherent risk of personal injury beyond the risks associated with many of the recreational activities at First Alliance Church: Toronto. I/we understand and accept these risks and agree that by allowing my child to participate in those activities, he/she may be taking part in a recreational activity that presents the potential for personal injury.
Medical Release*

I/we, the Parents or guardians named below, authorize the Pastor or one of First Alliance Church: Toronto personnel to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above in case of an emergency.
Release of Liability*

I/we, named below, undertake and agree to indemnify and hold harmless program personnel, First Alliance Church: Toronto and it's leaders and Board from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of First Alliance Church: Toronto, as well as of any medical treatment authorized by the supervising individuals representing First Alliance Church: Toronto. This consent and authorization is effective only when participating in or traveling to events sponsored by First Alliance Church: Toronto.
Photo Release*

From time to time First Alliance Church: Toronto may take pictures, videos, or make sound recordings during it's programming. These photos/videos, without identifying names or tags may appear during other First Alliance Church: Toronto events and/or through First Alliance Church: Toronto media and affiliated accounts (church website, upload sites, social media such as Instagram, Facebook, Discord, WhatsApp etc.). By enrolling your child, you are granting permission for your Child to be photographed and/or videotaped at different Children and Youth Programs.
Travel Permission

Transportation will be provided for all students by the church at no additional cost. The church will cover all transportation expenses, and travel will be fully insured. Parents/Guardians can be assured that every precaution will be taken to ensure the safety and wellbeing of each participant during travel to and from Joy Bible Camp.
Travel Release

I/we give permission for my child to be transported in a motor vehicle driven by First Alliance Church: Toronto approved drivers to the event above at the specified location on the date indicated. I understand that my child is expected to follow all applicable laws regarding riding in a motor vehicle and is expected to follow the directions provided by the driver and/or other adult volunteers. I/we understand that participation in the identified event is not a requirement for participation in First Alliance Church: Toronto’s activities.
I have read, understand, and discussed with my Child that:
1. They will be traveling in a motor vehicle driven by an adult and accompanied by a second adult and/or another child and they are to wear their safety-belt while traveling;
2. They are expected to respect each other, the vehicles they ride in, and the people they travel with during the trip;
3. Riding in a motor vehicle may result in personal injuries or death from wrecks, collisions or acts by riders, other drivers, or objects; and,
4. They are to remain in their seats and not be disruptive to the driver of the vehicle.
Purpose and Extent*

I understand First Alliance Church: Toronto is collecting and retaining this personal information for the purpose of enrolling your Child in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your Child, and to inform you of program updates and upcoming opportunities at our organization. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel.
