Thank you for your interest in Discovery Academy!

Please review the information below to apply for the 2026-2027 school year.

Application Instructions

You will have the opportunity to upload all required documents directly within the application. Before beginning, please gather the following items:
- Two proofs of residency
- If your child is an ESE student, submit a copy of the most recent IEP
- If your child has a 504 plan, submit a copy of the most recent 504 plan
While uploading directly is preferred, these documents may be emailed or submitted in person.

A separate application must be completed for each student. Upon submission, you will receive an email confirmation of your application form. Please keep this email as proof of submission.

Important Requirement and Information
1. Enrollment is limited by grade level and within specialized programs.
2. As a public charter school, Discovery Academy is open to all Polk County residents; however, transportation services may be limited. Please email betty.newman@discoveryacademy.org for more information.
3. The documents below may be uploaded within the application or emailed to: gissell.nava@discoveryacademy.org. Documents may also be submitted in person to the school.
  
  • Students applying from outside Polk County, out of state, or from private/online schools will need to provide additional information to complete the application. Please click here for additional information.
  
  • PROOF OF RESIDENCY – TWO proofs of residency MUST include the name and current physical address. Please click here for proof of residency requirements.
  
  • Students with an IEP or 504 plan are required to submit the most recent copy of that plan before the application can be processed.
4. After submission, you are required to inform the school of any changes to your contact information (address, email, or phone number). Since a change of address can affect your transportation status, please email betty.newman@discoveryacademy.org.

CONTRACT FOR PARENTS/GUARDIANS • GRADES 6-8

By choosing to enroll a child at Discovery Academy of Lake Alfred, parents provide their child with an exciting, enriching educational opportunity. Registration of a student indicates that parents or guardians have accepted the responsibility to provide the guidance, encouragement and home environment to foster the best possible learning situation. Discovery Academy of Lake Alfred expects the family to comply with the requirements of the school. Please read the following expectations carefully.

1. My child is expected to attend school every day, to arrive promptly and to remain throughout the scheduled hours. There will be no checkouts between 2:00 p.m. and 3:15 p.m. Excessive tardiness, checkouts and/or unexcused absences may result in my child being placed on attendance probation.

2. I understand that as a parent or guardian I must be supportive of my child and his/her school. I alsounderstand that I am required to attend parent conferences, school meetings and other activities sponsored by the school.

3. My child is expected to adhere to the strict uniform dress code and discipline policy. My child will take responsibility for his/her own behavior and will seek adult assistance when necessary. I understand that it is my responsibility as a parent to reinforce school expectations and communicate any concerns with school staff. Failure to follow school expectations may result in my child being placed on behavioral probation; committing serious breaches of the Code of Conduct could lead to dismissal from Discovery Academy of Lake Alfred.

4. I understand that my child is participating in a rigorous academic program and will maintain continuous academic progress and desirable work habits. Failure to maintain academic requirements may result in being placed on an academic success plan that will include mandatory parent intervention conferences.

5. I understand that my child could be placed on attendance or behavior probation for not meeting expectations of Discovery Academy of Lake Alfred. If my child is placed on probation, my presence as a parent or guardian is mandatory at a scheduled intervention conference to help develop strategies that address the issue.

6. I further understand that repeated offenses or failure to fulfill stipulations of the Discovery Academy of Lake Alfred Family Contract will result in my child being exited from Discovery Academy. If a recommendation is made to dismiss my child, I understand that I may appeal this recommendation according to the governing board’s approved Appeal Procedures Guidelines. Dismissal will preclude my child from priority status if reapplying.

7. Cell phones, cameras, portable gaming systems, smart watches & any other electronic devices should not be found on campus. If on campus, cell phones are required to be powered off and out of sight. They must be stored in a trapper keeper or clear backpack. Repeated offenses will be dealt with as insubordination and could lead to dismissal.

8. I understand that it is my responsibility as a parent/guardian to contact the school and provide current emergency contact information throughout the school year (i.e. change of address, phone number, email, etc.).
