    • Please read the directions carefully and fill out all forms completely.

     

    • All forms are to be submitted via electronic submission including any required documents such as immunization records no later than Thursday, February 5, 2026. Incomplete packets will not be accepted and registration is not complete until we have the required documents.

  • Note: An emergency contact person is an adult who can make decisions regarding a student when the parents cannot be reached and immediate action needs to be taken for injury or illness. They must present valid photo identification to Heritage Junior High School personnel if taking the student from school.

  • Authorized Drivers

    The following persons (including the "Emergency Contact Persons" listed above) are the only ones authorized to sign-out and pick-up our child from school if we (the parents) are not available. They must present valid photo identification to Heritage Junior High School personnel.

    information must be made by updating the Emergency Card online.

    To ensure student safety, we cannot accept changes made by phone or email.

  • Authorization for Consent to Medical Treatment of a Minor

  • Note: This consent form will be in effect from July 2025 to June 2026. It only applies for medical emergencies at school or for a school sponsored outing or field trip.

    The undersigned, parent/guardian(s) of *, a minor, do hereby authorize a designated representative of American Heritage Charter Schools , as agents for the undersigned to consent to an X-ray examination, an anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by, and is to be rendered under the general or special supervision of, any physician or surgeon licensed under the provisions of the Medical Practice Act or the medical staff of any hospital, whether such diagnosis or treatment is rendered at the office of said physician or at said hospital.

    It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care being required but is given to provide authority and power on the part of our aforesaid agent to give specific consent to any and all such diagnosis, treatment or hospital care which aforementioned physician in the exercise of his/her best judgment may deem advisable.

    This authorization is given pursuant to the provision of Section 25.8 of the Civil Code of California.

    We hereby authorize any hospital, which has provided treatment to the above named minor pursuant to the provisions of Section 25.8 of the Civil Code of California to surrender physical custody of such minor to our above named agents upon the completion of treatment. This authorization is given pursuant to Section 1283 of the Health and Safety Code of California.

    It is understood that primary medical cost incurred by the above named minor are to be covered by the parent’s or guardian’s insurance in case of any accident. A secondary source might be the insurance of the driver of an automobile in case of an automobile accident.

    We as parents or guardians, shall indemnify, hold free and harmless, assume liability for and defend American Heritage Charter Schools, its agents, servants, employees, officers, and directors from any and all costs and expenses including but not limited to attorney’s fees, reasonable investigative and discovery costs, court costs, and all other sums which American Heritage Charter School’s assertion of liability, or any claim of action founded thereon, arising or alleged to have arisen out of our (my) child’s use of real or personal property belonging to American Heritage Charter Schools, it’s agents, servants, employees, officers and directors or by actions of omission by our (my) child.

  • Note: If your student needs an EpiPen for an allergy, it is advised that one is kept in the office at school.

  • Medication Policy: 

    If your student requires any type of medication (including over-the-counter medicines), the school must have a completed MEDICATION AUTHORIZATION form on file in the school office. Students are NOT permitted to keep any medication except an asthma inhaler on their person. Note: If a student needs to carry an asthma inhaler, there must be a MEDICATION AUTHORIZATION form on file in the office.

    Asthma inhalers must be clearly marked with the student' name AND be in the prescription labeled box.

  • Student Information Release

  • Throughout the year, American Heritage Charter School and Heritage Elementary students may be highlighted in efforts to promote activities and achievements. American Heritage Charter Schools may seek to release identifiable student information in either print or electronic format to be utilized in either internal or external media sources that may be released to the public. This may include the student’s digital photograph, or video and/or audio recordings of the student. This material may be used in media publications, newspaper articles, television coverage, websites (including hjh.amhcs.org), social media, school yearbook, school newsletters (including those in electronic formats), video presentations, and/or school district or public presentations.

  • I    *  permission to release information described above about my student for use in AHCS OWNED and PRODUCED materials. Examples include school newsletters posted on AMHCS.org that contain student photographs, school webpage or other online formats, social media, audio recordings, video images, school educational or promotional materials, and graduation programs. Note: Student names WILL NOT be attached to student photographs on public forums such as school webpages or social media sites.

  • I   *      permission to publish images or written material about my student for use in the AHCS OWNED and PRODUCED school yearbook. PLEASE NOTE: IF YOU SELECT "DO NOT GIVE" PERMISSION, YOUR STUDENT WILL NOT APPEAR IN THE YEARBOOK.

  • I   *   permission to release information described above about my student for use in NON-AHCS OWNED/PRODUCED materials. Examples include information produced and broadcast by the news media on television and newspaper articles written and published by the local newspapers. 

  • Off-Campus School Activity Release

    2025-2026 School Year
  • Permission is requested for your son or daughter to participate in off-campus activites at Escondido Charter High School (ECHS) for educational activites using ECHS facilites.

     

    The activities will occur on school days from 8:00am-3:30pm.

     

    Students will walk with a staff member.
