I and/or my child, listed below in the applicant section of this form, has my permission to participate in the tryouts for Unity High Steppers. I understand that he/she must abide by the rules and regulations set forth by the It Takes A Village administration and the coaches of Unity High Steppers. I understand that my child must submit a tryout video or my child will not be considered for a high stepper position. I understand that my daughter/son will be evaluated by qualified judges, and we agree to abide by the decision of the judges. I understand by the very nature of the activity, that myself and/or my child are in the proper physical condition and capable or participating in the related activities or the drill team, understanding that ITAV is not in any way responsible for making such a determination. I agree on behalf of myself and/or my child, to release, hold harmless, and discharge ITAV from all claims, costs, liabilities, expenses, and judgments, including attorneys' fees and court costs for any occurrences in connection with any participation of this 2026 drill season. I understand that ITAV will not be responsible for any accidents and injuries (including up to death) to myself and/or my child while in the preparing for tryouts, practice and/or performance locations, where ITAV is housed or during a performance. I give my permission to ITAV staff, choreographers, and/or volunteers to take myself, or my child to a medical facility, if necessary. In case of emergency, if none of the contacts on file can be reached, I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine. This care may be given under whatever conditions are necessary to preserve life, limb, or well-being of myself and/or my child. I accept full responsibility for all cost of said medical care and any medical care or emergency treatments. I hereby waive all claims whatsoever in connection with such medical treatments. I understand that ITAV is not responsible for personal property that is lost, damaged, or stolen while at practices, activities or events of the Unity High Steppers Drill Team. I authorize and agree that ITAV may take and use photographs, videos, or likenesses of myself and/or my child as needed for its record-keeping, promotional use, advertising and/or public relations projects and that I have no rights to the same and will not be compensated for the same.
