I understand and will comply to the enforcement of the team's NO PHONE POLICY during working hours unless the Camp manager advises to use it for necessary purpose like filming or recording. Phones will be collected in beginning of day and be put into a team phone basket. Team will use personal laptops for work and I can bring my laptop for team work.I will respect my assigned serving time by coming punctually, and I will actively communicate this schedule or any time conflict to my leader ahead of time.I understand that my hiring can be terminated if I have any intolerable behavior or attitude towards anyone, or if there are evidences that my speech, my conduct, my faith and my purity is questionable that may stumble others, as stated 1 Tim 4:12 in the Bible (Don’t let anyone look down on you because you are young, but set an example for the believers in speech, in conduct, in love, in faith and in purity.)I understand use of drugs, alcohol, tobacco, foul language, bullying or any form of violence will NOT be tolerated and may result in termination of employment/volunteer service.No dating relationships within team members and campers are allowed during the camp.I will dress modestly for work and camp to reflect the beauty and purity as a Christ follower. No tight-fitting, revealing, deep or low cut, crop-tops or mini skirts are allowed for girls.I will avoid gossip about other children and team members. I will seek to actively communicate kindly and honestly for all needs to related people as early as possible. I can seek Camp Pastor and Camp Manager's help immediately whenever I need help.I will avoid forming cliques that divide the unity of camp. I will seek to be inclusive, encouraging and supportive to all.

Testimony Requirement: Please attach a written testimony of your salvation and walk with Christ as part of your application. Your testimony may include: How and when you accepted Jesus as Lord, Why you chose to follow Him, How your life has changed since, a description of your daily devotion practices, any spiritual struggles you are facing, how you continue to grow in your relationship with the Lord. Note: Applications without this testimony or other required documents will be considered incomplete and may not be reviewed.

Agreement and Consent

I give Peoples Gospel Church (PGC) officials the authority to act on my behalf in case of emergency. I authorize PGC officials or staff to sign a consent form for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for me. I am responsible for any expenses caused by emergency treatment or services in case of any accident. I agree that PGC will not be liable for any legal or other consequences resulting from any accidents.I undertake and agree to indemnify and hold blameless PGC, its personnel, Directors and Board from and against any losses, costs, damages or injury, harm or death suffered as a result of being part of the activities organized by PGC, as well as of any medical treatment authorized by the supervising individuals representing PGC. I assume ALL legal responsibility including any health care expenses incurred at a hospital or medical clinic in a medical emergency for injury or sickness for me. PGC accepts no responsibility for refusal of care if the Provincial health card information is not provided.I give permission for PGC to use my photographs, videos and sound recordings in any media form. I have read, understood and agree to the terms and above.Upon confirmation of hiring, all candidates must submit the following document to complete your application of Camp Counsellors: A police check record for Vulnerable Sector screening (if you’re born on or before June 30th, 2007). Regular PGC attendees will be reimbursed, please keep the original receipt.Upon confirmation of hiring, all candidates must go through and get a Valid CPR level C, Emergency (or standard) First Aid certificate.
