USTA League Team Confirmation & Captain's Agreement
A USTA League Team Confirmation & Agreement form is required for all USTA League teams. Captains need to complete the information listed below and sign the form. Captains must sign the form for the team to be included in the match schedule.
Completed team confirmations must be submitted by April 10th, 2026 to be included in the league season.

Minimum Number of Players Registered (7) by: 4/17/26

*Seven (7) players is the minimum for local play. Nine (9) is the minimum if you advance to nationals, please plan accordingly.

*Please do not apply for a Team Number unless you have a minimum number of players committed. If you need help finding more players, contact Stacy at sstephen@pnw.usta.com
