RELEASE OF LIABILITY In signing this form, I (Parent/Guardian) agree not to hold Braeswood Assembly of God Church, Inc., its officers, employers, or other agents liable for any loss, damage, additional expenses, accident, injury or death resulting from accidents, crime, sickness, weather, natural disaster or any other cause whatsoever that my student(s) listed in this registration form above might encounter while participating in church activities. I realize and acknowledge that my child's participation in any church activity includes risks and possible dangers. I am well aware that their travel exposes them to such risks as accidents, illness, and other calamities. I hereby assume any such risks that might result from my child's travel, and I hold Braeswood Assembly of God Church, Inc., its officers, employers, or other agents unconditionally harmless and blameless for any liability concerning my child's personal health and well-being, or any liability for their personal property that might be lost, damaged, or stolen while participating in church activities. I have carefully read the foregoing, and I understand that my signature herein holds Braeswood Assembly of God Church, Inc., its officers, employees, or other agents harmless for any liability for injury, damage, loss, accident, delay or irregularity in schedule.The undersigned being the lawful parent(s) and/or guardian(s) of the above-mentioned child and/or children hereby consent to the participation of said child and or children in the above listed trip conducted by Braeswood Assembly of God Church and to all the events related to this activity on the dates listed above. The undersigned hereby certifies that the child and or children are able to participate in the above-described activity without limitation unless listed above. The undersigned hereby further authorizes any of the employees, agents and/or representatives of Braeswood Assembly of God Church, Inc., its officers, employers, or other agents to (i) provide for, approve and authorize any health care at any hospital, emergency room, doctor's office or other institution; (ii) employ any physicians, dentists, nurses or other person whose services may be needed for such health care; (iii) review, and if necessary, disclose the contents of any confidential medical records; and, (iv) execute consent forms required by medical, dental, or other health authorities incident to the provision of the medical, surgical, or dental care to the child and/or children.The undersigned acknowledges completion of the health history section above.
