PLEASE READ THESE INSTRUCTIONS PRIOR TO COMPLETING THE FORM

In order to complete this form you will need to have the following documents ready to be uploaded at the appropriate time

- Information from Netball Learning (accreditation if you have any, exam pass details, Foundation course details, Intermediate Course detials)

- A completed copy of a ATO Statement from Supplier form -ATO STATEMENT BY SUPPLIER FORM - When completing the form the Supplier is you as the umpire.

If you are 16 or OVER:

You will need to complete this prior to registering!
You will need to upload evidence that you have completed the following course from Netball Australia

- Have a Photo of a Working with Child Check (or equivilent) if you are 18 or turning 18

I hereby declare and advise Essendon & District Netball Association (EDNA) that I will be providing my services to EDNA as a hobby and on a casual basis.

Any remuneration payable to me for this service should not be regarded as subject to PAYG Withholding Tax deductions.

I also advise that I am not a registered GST Supplier, therefore GST is not applicable to any remuneration payable to me for providing this service.

I agree to advise Essendon & District Netball Association immediately if there are any changes to my details or to this arrangement.
