Important note: with the help of the English translation on page 3 and 4, you need to fill in the *French* version of the document, i.e. the first paragraph on Page 1, then tick 'Autorise à' (which translates in 'Grant permission'), and then add date, name and signature of your legal guardian at the bottom of Page 2.
