WAIVER AND RELEASE OF LIABILITY
In consideration for my child being allowed to participate in the Legendary Kid's Camp, hosted by Peninsula Baptist Church at the Macon County Fairgrounds, I, the undersigned parent or legal guardian, agree to the following terms:
1. Assumption of Risk:
I understand that participation in activities at Legendary Kid's Camp involves physical activities, which may carry a risk of injury. I acknowledge that my child’s participation is voluntary, and I assume all risks associated with such participation, including but not limited to accidents, injuries, or illnesses that may occur during camp activities or while at the event location.
2. Medical Authorization:
In the event of illness, injury, or an emergency, I authorize the camp organizers, staff, or any other designated medical personnel to provide first aid or emergency medical treatment to my child. I also authorize the camp staff to arrange for transportation to a medical facility if necessary. I understand that every effort will be made to contact me in case of an emergency.
3. Medical Conditions and Allergies Disclosure:
I have disclosed all medical conditions, allergies, and special needs that may affect my child’s participation in the camp’s activities in the provided forms. I understand it is my responsibility to ensure that this information is current and accurate.
4. Release of Liability:
I, for myself, my child, and our respective heirs, executors, and administrators, hereby release and hold harmless Peninsula Baptist Church, the camp organizers, volunteers, employees, and agents, from any and all liability for any personal injury, property damage, or loss of any kind that may result from participation in the camp or from activities related to the camp, whether caused by negligence or otherwise.
5. Permission for Photos and Videos:
I grant Peninsula Baptist Church and its representatives permission to take photographs and/or video recordings of my child during camp activities. I understand that these images and videos may be used for promotional materials, including but not limited to websites, social media, newsletters, and other church-related publications.
6. Behavior Expectations:
I agree to ensure that my child follows the camp’s behavior expectations and rules. I understand that failure to adhere to the camp’s rules may result in my child being sent home at my expense.
7. Acknowledgment of Health Insurance:
I certify that my child has current health insurance, and I understand that I am financially responsible for any medical treatment or services required during the camp.

By signing this form online, I acknowledge that I have read and fully understand the contents of this waiver. I voluntarily agree to the terms and conditions outlined above and give my consent for my child to attend and participate in Legendary Kid's Camp.
