    This application is your first act of leadership in the fellowship—clear, intentional, and grounded in action. Please plan to spend 45–60 minutes completing the form.
  • Before You Begin

    We recommend reviewing the program information and attending an Info Session to ensure strong alignment between your goals and the fellowship design.

    What You’ll Need to Complete the Application

    Please have the following materials ready before starting the application:

    1. Brief CV / Résumé (PDF Upload)

    • Focus on your current role, leadership scope, and decision-making authority

    2. Letter of Support (PDF Upload) from a supervisor or organizational partner

    • Acknowledging that you will be participating as a fellow in this program
    • A sample letter is provided for guidance
    • You will also be asked to provide:
      • Employer and employer address
      • Supervisor name, phone number. and email address

    3. Two Professional References willing to complete an emailed recommendation form on your behalf

    • For each reference, you will need:
    • Name
    • Title and organization
    • Email address
    • Relationship to you

    4. Proposed Capstone Implementation Concept

    • As you begin the application process, start reflecting on the direction of your capstone project. Consider where in your community you might implement this work and what systems of support could sustain it. At this stage, you are not required to submit responses; this is an opportunity for thoughtful exploration and early visioning.

    Completing the Application

    • Answer all narrative questions thoughtfully and concisely
    • Be specific about what will change because of your leadership
    • If applying as part of a team (up to 3 people):
      • Each fellow must submit a separate application
    • To ensure clarity and consistency in the admissions process, please note that the application is available only in English and should be completed in English. Once admitted, however, participants are encouraged to use all available translation and accessibility features throughout the program.

  • In this section, we are not looking for a perfect answer. We want to understand how you think. Leadership can happen in many roles, in a classroom, a school, an organization, or a community. Please share a real challenge you are navigating or recently navigated, and explain how you made sense of it and what you decided to do next. It is completely okay if the situation is still unfolding or if you are still building confidence with this kind of thinking. What matters most is that you make your reasoning visible. Tell us who is involved, what might be shaping the challenge beneath the surface, where pushback might show up, how you will build support without losing trust, and what clear signs you will look for in the next few weeks to know you are making progress.

  • Name 2–3 observable indicators you’ll look for in the next 4–6 weeks to know your leadership is working (adult practice shifts, learner experience, system behaviors).

  • The next question helps us understand the context in which your capstone project would be implemented. Responses allow us to assess readiness, anticipate support needs, and design appropriate scaffolds so that capstone work is feasible, ethical, and aligned with your organizational responsibilities. There is no “right” answer—this information is used solely to support the successful and responsible implementation of the project.

  • Section 8 — Sponsorship and References

  • Please upload a brief letter of support from a supervisor, site leader, or organizational partner confirming awareness of your participation in the fellowship program. The letter should affirm the project's general scope and indicate that appropriate access (e.g., time, data, participants, or settings, as applicable) has been approved in accordance with organizational policies. This letter is not a legal agreement and does not require a detailed project plan; it serves to ensure feasibility, ethical alignment, and shared understanding.

    References must use the AMS Emerging Leaders Fellowship Program recommendation form. Below list the name of an individual who will provide a recommendation for you for this program. An email will be automatically sent.
