Admission decisions are based on a wide range of considerations and are solely and exclusively the right of Muneer Academy to decide.
The student’s records and information may be shared with the School Board’s healthcare partners as needed to provide and evaluate health care services.I acknowledge and affirm to abide to all school guidelines and rules outlined in the Parent and Student Handbook or otherwise stated.In case of emergency, when authorized people noted on this emergency contact card cannot be reached, school personnel have my/our permission to take whatever action is reasonable and appropriate under the circumstances for the welfare of my/our child.In case of emergency, the student may be transported by Emergency Medical Services to a hospital and provided treatment, and I am responsible for charges related to transportation and medical treatment.All information on this form is true and accurate to the best of my knowledge.I Agree to Terms and Conditions
