Divine Energy Healing

A multiphasic and multi-dimensional healing modality releases negative energy. It balances the energetic systems within the physical and spiritual bodies.

Energetic blocks can come in many forms, such as trapped emotions, mental/emotional/spiritual programs/patterns, attachments, cords, etc.

Refunds

Due to the nature of the energy healing sessions, there is a no refund policy for completed sessions.

Confidentiality

This energy-healing relationship and all information (documented or verbal) the Client shares with Teha Callen as part of the relationship is not legally confidential (like the medical and legal professions). Thus, communications are not subject to the protection of any legally recognized privilege. Teha Callen agrees not to disclose any information pertaining to the Client without the Client’s written consent.

Confidential information does not include information that: (a) was in Teha Callen’s possession prior to its being furnished by the Client; (b) is generally known to the public or in the Client’s industry; (c) is obtained by Teha Callen from a third party, without breach of any obligation to the Client; (d) is independently developed by Teha Callen without use of or reference to the Client’s confidential information; or (e) Teha Callen is required by statute, lawfully issued subpoena, or by court order to disclose; (f) is disclosed to Teha Callen, and as a result of such disclosure Teha Callen reasonably believes there to be an imminent or likely risk of danger or harm to the Client or others; and (g) involves illegal activity. The Client also acknowledges their continuing obligation to promptly raise any confidentiality questions or concerns with Teha Callen.

Cancellation Policy

Client agrees that it is the Client’s responsibility to notify Teha Callen at least 24 hours before the scheduled sessions. Teha Callen reserves the right to charge 50% of the session charge due to a reschedule within the 24-hour period or the full amount of the session for a no show.

Client Agreements

I understand that no guarantees have been made to me regarding the effect of such services.

I understand and acknowledge that in no way are these services to be construed by me as the diagnosis or treatment of disease or mental health diagnosis but rather as an aid to balancing my energy and possibly improving my physical, emotional, mental, and spiritual well-being.

I understand that energy healing is not a substitute for medical treatment, or medications. Therefore, it is recommended that I concurrently work with a qualified healthcare professional for any condition I may have.

I understand that I may experience improved physical, emotional, mental, and spiritual well-being. I am aware that Teha Callen does not diagnose illnesses and does not prescribe medical treatments. I understand that Teha Callen from time to time may recommend or refer me to a healthcare professional.

I understand that the work and methods during my sessions are proprietary. I will not attempt to perform this work on anyone else without the proper training. I also understand that resources are available to me through The Gestalt House LLC if I feel moved to do this type of energy healing work.

I understand that it is up to my discretion to act or not act upon any advice or intuitive information given to me during a session. I further understand that my life and the outcomes that occur in my life are based on the actions I choose to take or not take.

If I experience any discomfort during the session, I will immediately communicate that to Teha Callen so the treatment can be adjusted.

Release

I assume full responsibility for personal injury to myself. In exchange for such treatments, I hereby fully release and forever discharge The Gestalt House, LLC (including its officers, members, owners, employees and agents) from any and all damages, costs, expenses, liabilities, causes of action, claims and demands, of whatever character, in law or in equity, whether known or unknown, direct or indirect, asserted or unasserted, and whether or not on account of myself, The Gestalt House, LLC or other third parties, or in any way arising out of the above described treatment I have requested The Gestalt House, LLC perform. It is the intention of the parties that this agreement binds all parties whose claims may arise out of or relate to the treatment, or services provided by The Gestalt House, LLC including any spouse or heirs of the client/patient and any children, whether born or unborn. Any legal or equitable claim that may arise from participation in the treatment shall be resolved under Missouri law. I agree to indemnify, hold harmless and defend The Gestalt House, LLC (including its officers, members, owners, employees and agents) against all third-party claims, causes of action, damages, judgments, costs or expenses, including attorneys’ fees and other litigation costs, which may in any way arise from the above-described treatment I have requested The Gestalt House, LLC perform.

I understand this completed and signed consent for cover and apply to all present and future treatments herein mentioned.

I give Teha Callen permission to intuitively feel into my energy fields before my sessions if needed to identify the energies that need to be released.

By signing this agreement, I confirm that I am over the age of 18. I certify that I have read the above paragraphs; fully understand this consent and treatment form and herby consent to the indicated treatment(s). This means that I accept full responsibility for these and/or any other complications which may arise or result during or following the esthetic and massage treatments which is to be performed at my request according to this agreement and I hereby agree to arbitration of any malpractice claim. I further understand that by signing this agreement, I surrender certain legal rights.

Electronic Signature and Consent

I agree and understand that by signing this Consent Form, that all electronic signatures are the legal equivalent of my manual/handwritten signature, and I consent to be legally bound to this agreement.
