Your answers will be used to help improve the Youth Homelessness Response System (including access points, shelters, and prevention services). This survey is anonymous. Because we do not collect your name or identifying information, your responses cannot be used to match you with services. This survey is not a way to request or receive services—it is meant to gather feedback about services.
