I am the legal parent/guardian to "the Minor." I hereby grant permission for the Minor to volunteer at a Coppell ISD Education Foundation Camp. I understand and agree to the following terms and conditions:
Volunteer Activities:
The Minor will engage in volunteer activities at the Camp, which may include but are not limited to assisting with campers, participating in recreational activities, supporting camp staff, and helping with camp-related tasks.
Supervision:
I acknowledge and understand that the Minor will be under the supervision and guidance of authorized camp staff members throughout their volunteering period. The Camp will make reasonable efforts to ensure the Minor's safety and well-being during their time of service.
Health and Emergency Information:
I affirm that the Minor is in good health and physically capable of participating in the designated volunteer activities. I have provided accurate and up-to-date information regarding the Minor's medical conditions, allergies, medications, and emergency contact details, which will be made available to the Camp staff. In the event of an emergency, I authorize the Camp staff to seek and provide necessary medical treatment for the Minor.
Assumption of Risk:
I understand that there are inherent risks associated with volunteering at a camp, including, but not limited to, accidents, injuries, and exposure to natural elements. I acknowledge that the Minor's participation in volunteer activities is voluntary, and I assume all risks and responsibilities associated with their involvement.
Code of Conduct:
The Minor agrees to abide by the Camp's rules, policies, and code of conduct. They will treat fellow campers, staff members, and property with respect, and will refrain from engaging in any behavior that may endanger themselves or others, or disrupt the Camp's operation.
Transportation:
I understand that it is my responsibility to arrange transportation for the Minor to and from the Camp, and to notify the Camp in advance if any changes or arrangements need to be made regarding transportation.
Release and Indemnification:
I, on behalf of myself and the Minor, release, indemnify, and hold harmless the Camp, its directors, officers, employees, and volunteers from any liability, claims, damages, or expenses arising from or related to the Minor's participation in the volunteer activities, except in cases of gross negligence or willful misconduct.
I have read and understood the contents of this Parental Consent Form. I hereby authorize the Minor's participation in the volunteer activities at the Camp.
