OHL Scholarship Activation Request Form

I have read, understand and agree to terms & conditions outlined in the most recent OHL Scholarship Information Package (and additional documentation), which is always available to me at OHL Education Services Website.

⚠️ Important: I only complete this Scholarship Activation Request Form (SAF) once, at least 2 months prior to my first year of school and within my applicable Activation Deadline. If I do not activate within my activation deadline, I forfeit my OHL Scholarship.

This Scholarship Activation Form notifies OHL Education Services that I'm ready to access OHL Scholarship funds and allows them to verify my eligibility and calculate my benefits in time to authorize scholarship billing to my school. Activation verification can take up to one month.

OHL Scholarships are not automatically issued—I must follow the required process, complete all steps (in OHL Scholarship Information Package), submit the necessary documentation, and meet the deadlines.

Missing deadlines or not following the process carefully will delay, and may limit or even forfeit my OHL Scholarship Eligibility and associated funding.

The 3 Golden Rules of OHL Scholarship - 1. Activate Your Scholarship on Time, 2. Enroll and Maintain Full-time Student Status (3 courses each) during Fall & *Winter *semesters. 3. Attend School Full-time in Consecutive (Back to Back) Years. ⚠️Important: If I do not follow these three rules, I forfeit my scholarship eligibility.

Activation officially starts my "scholarship clock" and sets my 12-month academic cycle (either starting in January or August/September).

I am responsible for keeping all scholarship documents organized for easy access. 💡Stay Organized: Create a dedicated "OHL Scholarship" folder in your email or on your laptop to save your submissions, PDF uploads, and key documents.

Once you hit submit, the OHL Education Services team reviews your file to confirm your eligibility based on: Your Standard Player Agreement, and applicable Re-Sign or OA Agreements Total games played and training camps attended. Any other contract-specific details.

The Scholarship Outline: If/After your eligibility is confirmed, you’ll receive an Official Scholarship Outline via email. This is your master document—it shows a year-by-year breakdown of the “up to” amount available each year and your level of benefits. A copy of your Standard Player Agreement and associated agreements will be included in the email. 💡Stay Organized: Save Your Scholarship Outline in your dedicated "OHL Scholarship" folder.

It is my responsibility to understand the OHL Scholarship rules outlined in my Standard Player Agreement , applicable Re-Sign/OA Agreements and the OHL Scholarship Information Package and Additional Documentation.

STEP 2: Submit the applicable OHL Scholarship Letter Request Form (SLRF) - At least 2 months before your first class starts. Before you submit: Register at your school. Pay your deposit. Do not pay your tuition upfront. Submit the form applicable to your type of school/scholarship.
