To Cancel a Membership this form must be completed. This does not guarantee a cancellation. Early Cancellation Fee is $100. Any cancellation due to moving more then 60 miles from the Athletic Center or Medical Reasons must have documentation provided for those that would like the Early Cancellation Fee waived.
