The service you have elected to participate in implies a financial responsibility on your part. This responsibility obligates you to ensure payment in full for your fees.
Should the client not render payment at the time of service, he/she must pay off their remaining account balance before being seen for a successive session. Stone Grown Climbing LLC reserves the right to cancel successive treatment sessions until the delinquent account is paid in full. If the client does not continue to receive services and still has an outstanding balance he/she will have 30 days to pay off their remaining balance in full.
I have read the above policy regarding my financial responsibility to Stone Grown Climbing LLC for providing coaching services to me. I certify that the information provided is, to the best of my knowledge, true and accurate. I agree to pay Stone Grown Climbing the full and entire amount of all bills incurred by me.
