I am participating in a Workshop arranged by the Foundation for New York's Strongest, Inc., d/b/a the Sanitation Foundation ("SF").
I understand that SF may photograph and/or videotape participants, and I agree to appear in any and all media taken for this project unless I check the item below stating that I do not agree to appear in any media taken for this project. SF may use such media for any lawful purpose.
I hereby waive all claims I may have against SF, DSNY, and the City of New York, including their officials, employees, agents, affiliates, sponsors, and other representatives, in relation to my participation, and agree to release and hold harmless SF, DSNY, and the City of New York, including their officials, employees, agents, affiliates, sponsors, and other representatives, from any and all claims, demands, or causes of action which are in any way connected with my participation.
