Review the criteria to determine if an individual may be a good fit for New Leaf Legacy Living's transitional housing program.

    • Are open to structured oversight and community living
    • Are motivated to work toward housing stability and independence
  • Important Program Clarifications

  • • New Leaf Legacy Living is a housing-focused, non-clinical program
    • This is a membership-based shared housing model, not a lease or tenancy
    • Placement is not guaranteed and is based on safety, compatibility, and availability
    • Background and credit reviews may be conducted but are not strict and reviewed case-by-case
  • Next Steps for Referrals

  • If an individual appears to meet the above criteria, referral partners may submit a Case Manager Referral Form or contact us to discuss eligibility before referring.
