Referral Eligibility Overview
Review the criteria to determine if an individual may be a good fit for New Leaf Legacy Living's transitional housing program.
New Leaf Legacy Living provides structured, transitional shared housing for adults seeking stability during a period of transition. This overview helps referral partners determine whether an individual may be a good fit for our program before submitting a referral.
Who May Be Eligible
Individuals must meet all the following criteria:
• Be 18 years of age or older
• Be able to live independently in a shared housing environment
• Have verifiable income, benefits, stipend, or placement plan
• Be willing to follow house rules and accountability standards
• Be seeking transitional housing, not permanent placement
We commonly serve:
• Veterans
• Returning citizens
• Young adults aging out of foster care
• Interns and working professionals
• Individuals with fixed incomes (SSI, SSA, VA benefits)
• Individuals seeking a fresh start due to life transitions
Program Fit Considerations
This program may be a good fit for individuals who:
• Can manage daily self-care independently
• Are not in need of medical, mental health, or substance abuse treatment services
• Are open to structured oversight and community living
• Are motivated to work toward housing stability and independence
Important Program Clarifications
• New Leaf Legacy Living is a housing-focused, non-clinical program
• This is a membership-based shared housing model, not a lease or tenancy
• Placement is not guaranteed and is based on safety, compatibility, and availability
• Background and credit reviews may be conducted but are not strict and reviewed case-by-case
Next Steps for Referrals
If an individual appears to meet the above criteria, referral partners may submit a Case Manager Referral Form or contact us to discuss eligibility before referring.
Our goal is to ensure safe, stable housing placements that support long-term success for both residents and property partners.
