I hereby submit my application to be a referee in the 2026 Dallas Open Taekwondo Championships on March 14-15, 2026, at Loos Field House (3815 Spring Valley Rd. Dallas, TX 75001). I agree to waive all claims against any persons connected to the said event for injuries I may sustain. I assume full responsibility for all my actions in connection with the said event. I understand that any pictures taken of me participating in the said event may be used for publicity without compensation. The 2026 Dallas Open Taekwondo Championships practices a NO REFUND policy. By signing below an agreement to these terms. I will follow all the rules and regulations set forth by the organizers of this event and conduct myself in professional and fair manner throughout this competition
