Class Selection
By signing this form, I acknowledge that I have applied to participate in class or classes selected above using the facilities and equipment of Dog’O’Pogo Agility. As a condition of the use of Dog’O’Pogo Agility Association's facility and equipment, I have assumed all risk of personal injury, death or property, and release Dog’O’Pogo Agility Association, its instructors, employees, and facility owners, and waive all claims with respect thereto.

In recognition of the possible dangers connected with any physical activity and involvement of any kind with dogs, I hereby knowingly and voluntarily waive any right or cause of any actions of any kind whatsoever arising as a result of such activity which any liability may or could accrue to Dog’O’Pogo Agility and their officers, directors, agents, facility owners, or instructors, their assigns or successors.
