Administration of First Aid and/or Medication. By checking the boxes below you are hereby authorizing the Directors and Staff representing Lazy Acres to provide first aid, and provide the following medication, or their generic equivalent, in accordance with the recommended dosage and intervals of administration. All Staff and Director are CPR and First aid certified. The Lazy Acres Director is also certified and has taken educational classes on Child development, child protection, seizers, epilepsy, diabetes and food safety illness and allergies.

Lazy Acres is a Christian based day camp. We believe in The Golden Rule, “Do unto others as you would have them do unto you”. We encourage kindness and fairness towards others and believe every child is entitled to a fun and safe environment at Lazy Acres! The following list is the type of behavior that we expect all children to follow. ●Respect for all campers and staff ●Participation in all appropriate activities ●Possess positive and caring attitude ●Follow proper safety procedures at all times. Disciplinary Plan - We believe all children are unique in their thoughts and actions, and will therefore determine the proper course of action based on each situation. We will include the parent when we feel it is necessary. Lazy Acres cannot serve children who display continued unacceptable behavior. If camper behavior is deemed a danger to themselves or others, said camper will be sent home immediately with no refund. Initial disciplinary write ups for “minor infractions”- For minor infractions at camp your child will receive a writeup and talk with one of our directors about our expectations at camp. A parent will be called about the situation if we feel the action requires them to be picked up for the day from camp. After three “minor infraction” write ups your child will be permanently dismissed from camp. We reserve the right to dismiss campers with no refund in the event of behavioral problems that may put the safety and well-being of Lazy Acres campers and/or staff at risk. REFUNDS ARE NOT GIVEN when campers are suspended or removed for unacceptable behavior. Examples of unacceptable behaviors include, but are not limited to, the following. Please check boxes below to acknowledge that you have spoken with your camper(s) about the behaviors listed and that they understand these behaviors are not tolerated and may result in immediate consequences, including early pick-up, suspension, or permanent dismissal, even on the first infraction.

The State requires all children admitted to a summer camp have submitted shot records or a notarized exemption affidavit. Your registration is not complete and your child is not enrolled in camp until we have received these forms. Please upload them below.
