Instructions: You can partially complete this application and return to work on it later.  Be sure to frequently use the "Save" button at the bottom of the form. You should do this every time you work on the application. You may jump around the application as it does not need to be completed in order. However, you must do Section 1 first and enter a valid email in order to save your application.

The first time you save you will get a message from Jotform with a link to return to your application. The link is resent every time you save. If you lose it, your info will be forever lost. Getting a Jotform account before you begin the application may help prevent loss of data. The first email will have a link to sign up.

You should use your own personal computer to complete the application. If you use a public computer your information may be visible to other users and they can change it. You must always use the same computer to enter or upload data.

When the form is complete you should use the green "First Print-Save as PDF" button at the bottom of the page. Select "Save as PDF" as your printer choice. Name the file like this, using your own name: Last name, First name, Middle Initial BECA 2026.pdf. You will then upload this pdf, along with a single pdf of your supporting documents using the yellow "BROWSE FILES" buttons at the bottom of this application. You must do this before you use the "Last step: Submit" button.

The pdfs of your completed form and supporting data, are due to BECA no later than midnight, Friday, March 6, 2026. Schools and public libraries have scanners you can use to scan your Supplemental Information.
