This form is used as a submission for additional documents. Please gather all documents for submission to your tax preparer.

Please note that this form is used only for documents that were left off during the initital intake process. If you decide to use the services of Lux Tax Brand & Co., LLC you will be responsible for submitting all documents needed to process your tax return.

Please be sure to click the “Submit” button at the bottom of the form. You will receive a confirmation email to confirm your submission.

This intake form is highly secure and all requested information is required by the IRS. Your information is securely transmitted and protected using SSL (Secured Socket Layer) which provides the same protective measures as banks and physician’s offices. Protecting your sensitive information is of high importance to our firm.
