I have made these arrangements myself and I accept responsibility for my child's transportation.

I will not hold HPSD liable for any accident or injury that may occur during the transportation to/from the off-site activity.

My child, will not transport any other student whom he is not authorized to transport. Authorization would be in the form of written consent from the student's parent.

I accept that HPSD's student accident insurance does not apply during the transportation portions of the trip, and the student is only covered from when he arrives at the school sponsored off-site activity, until he/she leaves the off-site activity.
