By signing below, I acknowledge and agree to the following:

1. Voluntary Service

I am voluntarily choosing to receive permanent jewelry services from For Keeps LLC.

2. Health & Safety

I confirm that I do not have any known medical conditions, allergies, skin sensitivities, or other health concerns that would make receiving permanent jewelry unsafe. I understand it is my responsibility to disclose any concerns before the service begins.

3. Risks

I understand that permanent jewelry involves a small welding process and that there is a minor risk of:

• Skin irritation
• Allergic reaction
• Burns
• Discomfort during or after application
• Damage to clothing or personal property

4. Aftercare

I understand that proper care of my jewelry is my responsibility. For Keeps LLC is not responsible for damage, loss, stretching, or breakage that occurs after application.

5. Removal

I understand that permanent jewelry may need to be removed for medical procedures, employment requirements, security screenings, athletic activities, or other personal reasons. 

6. Release of Liability

I release and hold harmless For Keeps LLC, its owners, employees, and representitives from any claims, injuries, damages, losses, or expenses arising from my participation in this service, except in cases of gross negligence or intentional misconduct.

7. Minors

Clients under 18 years of age must have a parent or legal guardian present to sign this waiver.

8. Jewelry Material Acknowledgement

I understand that For Keeps LLC offers different jewelry materials, including, but not limited to, gold-filled, sterling silver, and stainless steel. I have selected my preferred material and understand that individual skin reactions and wear patterns may vary.

9. No Guarantee Clause

While we strive to provide high-quality jewelry and application, no guarantee is made regarding the lifespan of the jewelry due to individual wear, lifestyle and care.
