My child is physically fit, healthy, and able to safely participate in football training and related physical activities at The Futbol Academy Pty Ltd.
To the best of my knowledge, there are no medical conditions or injuries that would prevent my child from participating in these sessions.
I understand and accept that while all reasonable care will be taken, The Futbol Academy Pty Ltd is not liable for any injury sustained during participation in training sessions or related activities.
Social Media Waiver
By ticking the box below, I acknowledge that:
The Futbol Academy Pty Ltd may occasionally capture photos and videos during training sessions and events for use on social media, website, and promotional materials.
I consent to my child being included in such content, with no compensation due.
If I do not wish for my child to appear in any media content, I will notify the academy in writing by emailing hq@thefutbolacademy.com.au before their first session.
Payment Terms & Commitment Agreement
By ticking the box below, I acknowledge that:
An invoice will be issued upon submission of this registration form and must be paid within 7 days.
My child's place in the program is not secured until full payment has been received.
I am enrolling my child for the full term of sessions, not on a casual or week-to-week basis.
Missed sessions, regardless of reason, are non-refundable, and I remain liable for the full term fee.
Late payments may incur a 10% late fee at the discretion of The Futbol Academy Pty Ltd.
