Crucian Christmas Festival 2026-27 Season Registration

Majorette & Drum Major Team

1. No unfavorable behavior (It will not be tolerated). 2. No obscene language. 3. Members are expected to wear the practice uniform and sneakers to practice at all times. A fee of $2.00 will be charged if Member is not in practice uniform. 4. Members are required to participate in warm-up stretches and exercises before and after practice, to prevent injury. 5. Members should adhere to the practice schedule or notifications and arrive to practice within a timely manner.

I hereby give my consent and approval for my child to participate in all practices, rehearsals, parades, performances, travel, and related activities organized by Sparkle Lite Twirlers, INC., including but not limited to the Crucian Christmas Festival, St. Patrick’s Day Parade, and other sanctioned events. I understand that participation in majorette and drum major activities involves inherent risks, including but not limited to physical exertion, use of equipment, environmental exposure, travel, and performance-related activities. These risks may include, but are not limited to, sprains, fractures, paralysis, serious injury, or death. I voluntarily assume all risks and hazards incidental to my child’s participation and release, waive, discharge, and hold harmless Sparkle Lite Twirlers, INC., its officers, directors, coaches, instructors, agents, volunteers, affiliates, sponsors, advertisers, and representatives from any and all claims, demands, actions, or liabilities arising from injury, illness, loss, or damage sustained during or in connection with participation, including travel to and from events.

I hereby waive any and all claims against Sparkle Lite Twirlers, INC., including all coaches, affiliates, participants, sponsoring agencies, advertisers, and, where applicable, owners or lessors of premises used for practices or events.I further agree to indemnify and hold harmless Sparkle Lite Twirlers, INC. from any claims, damages, or expenses (including legal fees) arising from my child’s participation in practices, events, carnivals, parades, or activities.

In the event of illness or injury, I authorize the President/Director, Secretary, Treasurer, Branch Chiefs, or designated representatives of Sparkle Lite Twirlers, INC. to obtain and administer emergency medical treatment for my child, including contacting emergency medical services or transporting my child to a medical facility if deemed necessary.I understand that every reasonable effort will be made to contact me immediately in the event of an emergency.

I acknowledge and agree that: All parade and event expenses must be paid in full by the final payment deadline. Once uniform orders are placed, all payments are non-refundableSparkle Lite Twirlers, INC. is not responsible for loss, theft, or damage to personal belongings, including but not limited to batons, whistles, sneakers, jewelry, uniforms, or majorette/drum major equipment

I understand and acknowledge that this registration also serves as a Photo and Video Release Waiver. By completing this form, I grant permission for Sparkle Lite Twirlers, INC. to photograph, video record, and use my child’s image and likeness for organizational, promotional, marketing, educational, and social media purposes, without compensation.

Registration Fee (Non-refundable): $44.00. Baton Fee (Ordered Non-refundable: $40.00. The members’ parade expense will be communicated to parents in a separate correspondence once the Crucian Christmas Festival uniform has been selected.

I certify that I have read, understand, and agree to the terms of this Parental Agreement, including the Informed Consent, Release of Liability, Waiver and Indemnification, Medical Authorization, Financial and Property Acknowledgment, Member Fees, and Photo, Video, and Media Release. I voluntarily consent to these terms on behalf of myself and my child.
