Terms of Agreement: I understand and agree to Fo Guang Shan Monastery (FGS) and all affiliated branches and departments, may collect, process, and use the personal data that I have provided, including previously provided and provided in the future, for merit registration, ceremonies, activities and such as notification of activities, accounting processing and reporting of merits. In addition, the personal information of others provided by me, have been informed and agreed that the personal information will be provided to the relevant units of Fo Guang Shan Monastery and all affiliated branches and departments for collection, processing or use. *I have read and agree to the above terms.
協議條款：本人在此同意授權多倫多佛光山拍攝並使用本人之肖像，包括照片（數位或印刷，以下統稱「圖像」）及影音資料（視頻、音頻、MP3，以下統稱「影音像」），作為寺院出版物、宣傳及展示之用途。本人理解並同意，佛光山及其所有附屬道場與相關單位，得蒐集、處理及使用本人所提供之個人資料（包括過去已提供及未來將提供者），以作為功德登記、法會及各類活動之需要，例如活動通知、會計處理及功德申報等。此外，本人亦已告知並取得相關個人之同意，允許佛光山及其所有附屬道場與相關單位，蒐集、處理 及使用本人所提供之他人個人資料，以符合上述相關用途。
WAIVER OF LIABILITY: I hereby agree to release and forever discharge, Fo Guang Shan Temple of Toronto (also known as “I.B.P.S. of Toronto") its respective Buddhist monastics, instructors, staff and volunteers from all liability, whether direct or indirect, and hereby waive all claims, demands expenses, actions and cause of action which may arise from any injury to myself, and any loss or damage to personal property arising from, or in any way resulting from, my participation in the class(es).
責任免除聲明：本人在此同意並確認，永遠免除多倫多佛光山（又稱“I.B.P.S.Toronto”）、其所屬之法師、講師、工作人員及義工對本人不承擔任何直接或間接責任。本人放棄因參加課程所引起或以任何方式相關之事件，對本人造成的任何人身傷害、個人財產之損失、或由此而生之索賠、損害、要求、訴訟及法律行動。
