Please answer fully so we can get to know a little bit about your family and needs in a GSD and if a dog from our program might be a good fit for your needs. Please complete all questions, incomplete questionnaires are discarded.
Please tell us about your household. Include names, ages, and occupation (if applicable). Please also include family members who may reside with intermittently, or who visit regularly.
Are any members of your family allergic to dogs? Are any members of your household afraid of, or uncomfortable around, large dogs? How does each family member feel about adding a puppy who will grow into a large hairy dog to the family?
Who is going to be your dog’s primary caregiver? Does this person work full time? If the primary caregiver is a husband to a wife, how does the wife feel about adding a dog?
Do any members of this household have any special needs, either physical or developmental, that might affect how they relate, handle, or live with a large dog? Are you looking for a service dog prospect? If so what specific tasks?
Are there other pets living in your household (or who visit regularly)? If so please list each one along with species, age, and how you expect them to get along with a GSD.
Please tell us about your lifestyle. Include average work week (remote or in-office) for all adults in residence, after school commitments children attend, how you enjoy spending your spare time, and how much spare time you typically have.
Do you plan on taking training classes with your dog? If 'no' - how do you plan on teaching your puppy what it needs to learn? If 'yes' - please list the company you have chosen, if the class is in-person, online, or "board and train," and what classes they offer that most interest you. What attracted you to this service provider?
Do you plan on pursuing any dog sports such as: Agility, Obedience, Scent Work, Tracking, Rally Obedience, Schutzhund or Service Dog Task Training? If yes, please give the name of coach, club, or business you are planning on training with (this is required for all bite sport and Service Dog prospects.) How long have you been training with this club/coach?
Do you believe a dog needs to receive physical corrections (collar corrections, e collar stim, scolding, leash pops, "boops" etc) or punishment in order to be a well trained companion or sport dog? How do you typically correct your dog?
Do you plan on utilizing training tools such as a pinch/prong collar, choke/training collar,shock/e-collar, penny cans, spray bottles, to train your dog? What type of equipment do you currently own and prefer to use to train your dog?
Have you used such tools in the past to train or manage your dog, and do you plan on using them with future dogs? Have you received training in the proper use of these tools?
Do you believe that dogs will attempt to dominate people? If so, how do you plan on preventing this?
Do you live in an apartment or house? Do you rent or own? If you live in a house does your yard have a fence ( describe type and height)? Does your house have, or are you planning to install, a "hidden fence" type system?
