We’re so excited to kick off another year of OLOL Flame Teens! Whether you’re returning or joining us for the first time - welcome!

Our ministry has two programs:
- Teens (Years 7-9): Every Saturday from 4-6 PM
- Come Alive (Years 10-12): Every fortnightly Wednesday from 7-9 PM
This form is for both Teens and Come Alive and must be completed by a parent or guardian.

Parental Consent & Agreement
I give permission for my child(ren) to fully participate in OLOL Flame Teens at Our Lady of Lebanon Co-Cathedral, including regular sessions, group discussions, events, and off-site excursions where applicable.

I understand that every effort will be made to contact parents/guardians in the event of a medical emergency. If I cannot be reached, I give permission for the supervising leader to seek medical assistance and arrange necessary treatment for my child.

I acknowledge that:
- OLOL Flame Teens (Year 7-9) runs on Saturdays, with Leader supervision provided between 3:30 PM and 6:30 PM. It is my responsibility to drop off/pick up my child(ren) or ensure a trusted guardian does so on my behalf at the start and end of each session.
- Come Alive (Year 10-12) runs fortnightly on Wednesdays, unless otherwise advertised, with Leader supervision provided between 6:30 PM and 9:30 PM. I understand that I, or a nominated guardian, must arrange for my child(ren)’s drop-off and pick-up.
I consent to my child(ren) being photographed or recorded for official social media platforms, including OLOL Flame Teens, the main OLOL Parish platforms, and the church website/newsletter.

I acknowledge that OLOL Flame Teens uses official communication platforms for notifications, which may not always be responded to. However, I understand that the team can be contacted via:
