THE WALLACE FOUNDATION

The Wallace Foundation was created to provide awareness about physical, mental, and emotional health. To instill confidence, an individual sense of responsibility, and that each person recognize the need to have a plan for a future filled with life-enhancing goals, and for them to realize their full potential in great health.

In partnership with 29:11 Mentoring Families, our MLK Project 2026 is a community-wide Day of Service focused on restoring, beautifying, and supporting 29 homes along North 15th Street. Volunteers will serve through hands-on outdoor work, light home improvements, and food preparation and distribution as we work together to strengthen our neighborhood and uplift our community.

I hereby release and hold harmless The Wallace Foundation, 29:11 Mentoring Families, its employees, officers, agents, and affiliates from any and all claims, liabilities, demands, or causes of action arising out of or related to my participation in volunteer activities.
I understand and agree that this release is binding upon me, my heirs, assigns, and legal representatives.

By signing below, I hereby grant The Wallace Foundation & 29:11 Mentoring Families permission to capture and use my image, likeness, and voice in photographs, videos, or other media taken during volunteer activities. I understand these materials may be used for promotional, educational, or informational purposes, including but not limited to social media, websites, publications, and presentations.

I waive any rights to compensation or ownership of the photographs or recordings and release The Wallace Foundation & 29:11 Mentoring Families from any liability related to their use.
