Data Privacy- Consent Form
The Integrated Bar of the Philippines (IBP) recognizes its responsibilities as the personal information controller under the Republic Act No. 10173 (Act), also known as the Data Privacy Act of 2012, with respect to the data we collect, record, organize, update, use, consolidate or destruct/dispose from its members. The personal data obtained from this form is entered and stored within the Organization’s authorized information and communications system and will only be accessed by the IBP and IBP authorized personnel. The IBP has undertaken organizational, technical and physical security measure to ensure the protection of the IBP Members’ personal data.
The information collected and stored in this form shall only be used for the purpose of related to the processing of this form such as, but not limited to, recording of information for statistical data, coordination and other relative purposes reasonably inferred from the Program/Project of the IBP.
IBP undertakes to safeguard and prohibit any disclosure of its members’ personal information without their consent.
I have read IBP’s Data Privacy Statement and express my consent for the IBP, the IBP National Center for Legal Aid (IBP NCLA), and its Chapters to collect, record, organize, update or modify, retrieve, consult, use, consolidate, block, erase or destruct my personal data as part of my information.
I hereby affirm my right to be informed, object to processing, access and rectify, suspend or withdraw my personal data, and be indemnified in case of damages pursuant to the provisions of the Republic Act No. 10173 of the Philippines, Data Privacy Act of 2012 and its corresponding Implementing Rules and Regulations.
