    Important Notes

    • Dress appropriately.
    • No food or drink in the gym.
    • Keep the gym clean.
    • Waivers must be signed prior to playing.
    • No cursing/foul/derogatory language.
  • LIABILITY & MEDICAL RELEASE

    Although the safety of all sport activities is the primary concern, indoor sport activities at rental facilities may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against the rental facility and the persons in charge.

    I understand that participation in the Skills Camp and/or Indoor 3-on-3 Basketball Tournament involves physical activity and inherent risks, including but not limited to bodily injury. I voluntarily assume all risks associated with participation.

    I hereby release, waive, and hold harmless the tournament organizers, facility owners, sponsors, volunteers, officials, and staff from any and all claims, demands, injuries, damages, or liabilities arising from participation in this event.

    I certify that I am physically capable of participating and have not been advised by a medical professional to refrain from physical activity.

    In the event of injury or medical emergency, I authorize tournament staff to obtain medical treatment on my behalf.


    PHOTO & VIDEO RELEASE AUTHORIZATION
    I grant permission to the tournament organizers and their representatives to photograph, record, and/or videotape me during participation in the Skills Camp and/or Indoor 3-on-3 Basketball Tournament.

    I understand that these images, audio, or recordings may be used for promotional, marketing, social media, website, and other lawful purposes, without compensation.

    I waive any right to inspect or approve the finished product or the manner in which it may be used.

    For any participant considered a minor, a parent or legal guardian acknowledges and grants permission for photography and video recording of the minor participant.


    ACKNOWLEDGMENT & AGREEMENT
    By registering and/or participating in this tournament, I acknowledge that I have read, fully understand, and voluntarily agree to this Liability, Medical, and Photo/Video Waiver.

    If the participant is a minor, this waiver must be read and signed by a parent or legal guardian, who agrees to all terms on behalf of the minor.

    All participants agree to:

    1. Respect referees, staff, and facility rules
    2. Maintain sportsmanlike conduct at all times
    3. No fighting, taunting, profanity, or excessive arguing
    4. No alcohol, drugs, or weapons inside the facility.
    5. Violations may result in immediate removal and team disqualification without refund.
