For the Scholar Award, I affirm the student being nominated meets the following requirements (check all that apply):*

Student holds a GPA of 3.5 or higher (3.75 for Doctoral Scholar Award) in a 4.0 GPA scaleStudent is expected to graduate in the 2026 calendar yearStudent is earning a Kinesiology-related degree
For the Writing Award, I affirm the student being nominated meets the following requirements (check all that apply):*

The student is first author on the publicationThe manuscript was published in the 2025 calendar yearThe student was a current student in a Kinesiology-related graduate program when the manuscript was accepted for publication
Student Vita or Publication Upload
For Scholar Award: Submit a single pdf file containing the student's vita (not to exceed two pages) that includes the following information:
- Student's cademic performance,
- Scholarly interests/accomplishments,
- Leadership, and
- Service to the profession.
Note: If a student's vita exceeds the two-page limit, they will not be considered for the national award.

For Writing Award: Submit a pdf of the student's publication (CV is not required).

Please use the following naming convention when submitting your uploaded file: Please type your full instituion name as you did in the form above.
