Faith Mountain Academy Admission Application

  • Statement of Faith:

The Statement of Faith does not exhaust the extent of our Ministry’s beliefs. The Bible itself, as the inspired and infallible Word of God that speaks with final authority concerning truth, morality, and the proper conduct of mankind, is the sole and final source of all that we believe.

For purposes of our Ministry’s faith, doctrine, practice, policy, and discipline, our Board of Directors is the final interpretive authority on the meaning and application of the Bible.

We believe in God as He has revealed himself in the Bible. "Hear, O Israel, the LORD our God, the LORD is one." God interacts with His creation as the Father working through the Son and in the power of the Spirit. (Genesis 1:1; Deuteronomy 6:4; Mark 12:29; 1 Corinthians 8:6; Ephesians 4:4-6)
We believe that Jesus Christ is the Son of God, the Eternal One in whom all the fullness of deity dwells in bodily form, and who is the Word who became flesh and dwelt among us, and whose glory we beheld, the glory of the uniquely begotten Son of God, full of grace and truth. (John 1:1-14; Colossians 2:9)
We believe in His virgin birth, in His sinless life, in His miracles, in His death that paid for our sin through His shed blood, in His bodily resurrection, in His heavenly ascension to the right hand of the Father and in His personal return in power and glory. (John 1:1; Matthew 1:18,25; Hebrews 4:15; Hebrews 9:15-22; 1 Corinthians 15:1-8; Acts 1:9-11; Hebrews 9:27-28)
We believe that the Holy Spirit of God comforts, teaches, leads, indwells and empowers all whom God regenerates. (Acts 9:31; 1 John 2:27; John 16:13; 1 Corinthians 3:16; 2 Timothy 1:7)
We believe the Bible is the inspired, infallible, and authoritative Word of God and that “every Scripture is God-breathed and profitable for teaching, for reproof, for correction, and for instruction in righteousness, that each person who belongs to God may be complete, thoroughly equipped for every good work. (2 Timothy 3:16-17)
We believe that all have sinned and have fallen short of the glory of God. (Romans 3:23)
We believe the death and resurrection of Jesus accomplished the atonement for all who place their faith in him. Whoever trusts in God, in his work alone, is made a new creation in Jesus, indwelt by the Spirit of God, and is transferred from the kingdom of darkness into the kingdom of his own Son. (Ephesians 2:8-9; Romans 5, 6)
We believe in justification by faith through the grace of God alone, and not by human efforts. One may not earn, merit, or keep this eternal salvation by his own efforts. A new creation is the work of God alone. (Ephesians 2:8-10; Romans 8:29-39)
We believe in the spiritual unity and equality of all believers in Jesus Christ. (Ephesians 2:11-22)

    Objections to Medical Mandates:

While we are dedicated to our mission of spiritual growth, we also believe in the importance of individual liberty and parental rights. We respect the diversity of opinions on medical mandates and believe that parents should have the freedom to make informed decisions regarding their children’s healthcare, including vaccinations, in accordance with their faith and convictions.

  • The purpose of Faith Mountain Academy is to assist parents in providing an excellent education based in the Christian faith. As such, we ask that at least one parent believes in Jesus as the Savior. Please describe your relationship with Jesus Christ for each person below:

  • The training and schooling of children is essentially spiritual in nature, whether it is mathematics, science, literature, or any other academic endeavor. In order for the school to serve well as an extension of your home, it is importanta that the home and school share common spiritual beliefs and goals. To assist us in discerning how we can best serve in this way, please tell us your beliefs by responding to the following questions:

  • Does your familiy regulary attend church?*
  • Tuition Payment:

Since Faith Mountain Academy has financial obligations to its employees, students, and community, students are considered enrolled for the entire school year. Because the school’s expenses are incurred on an annual basis, the school cannot release parents from their financial obligation once enrollment is finalized.

I understand and agree that once the retainer fee has been paid, we (the parents/guardians) are responsible for payment of the full tuition and all applicable fees for the entire academic year.

Extenuating circumstances must be submitted in writing to the Board of Directors for review. Upon consideration, the Board may, at its discretion, release a family from its remaining financial obligation. All appeals are subject to review and approval. Student records will not be released or forwarded to another school until all financial obligations have been satisfied.

If a student enters after the school year has begun, tuition and fees will be prorated based on the actual number of days enrolled. No deductions or refunds will be made for tuition during the school year for absences, regardless of the reason.

If legal action is required to collect unpaid tuition or fees, the undersigned agrees to be responsible for all reasonable attorney’s fees and associated costs.

  • Ministry Commitment and Community Partnership

We affirm our commitment to Faith Mountain Academy as a Christ-centered, nonprofit ministry and acknowledge that the school depends on faithful stewardship and sacrificial giving beyond required tuition. As Scripture reminds us, “Each of you should give what you have decided in your heart to give, not reluctantly or under compulsion, for God loves a cheerful giver” (2 Corinthians 9:7). Such generosity enables the ministry to meet needs beyond standard operational expenses and to continue the work God has called it to do.

I also understand that, as a small Christian school community with limited staff, Faith Mountain Academy relies on the willing service and shared responsibility of the families it serves. In accordance with biblical principles of service and unity—“Now you are the body of Christ, and each one of you is a part of it” (1 Corinthians 12:27)—I am willing to support our school community in whatever ways the Lord enables me. This may include, but is not limited to, volunteering in my child’s classroom, assisting during lunch or recess, offering services or skills, and supporting the ministry through prayer and financial giving.

  • Discipline:

The word "discipline" means to make a disciple. This involves both the intrinsic and extrinsic aspects of teachign a child to live properly before God and in all spheres of life. A change in heart (intrinsic) is accomplished only by prayer, but structure, boundaries, and natural consequences help develop habits of discipline.

I understand that enrollment at Faith Mountain Academy is a privilege. The Academy exists to partner with families in the formation of character by training Christian youth in sound habits, self-discipline, personal responsibility, integrity, and good citizenship.

I believe discipline is an education in habits and is essential to the well-being of each student and the school community. As a parent, I agree to support the Academy’s discipline policies, reinforce these habits at home, and work cooperatively with faculty and staff.

  • School Property:

I understand that I am responsible to pay for damage, whetehr intentional or unintentional, to school facilities, equipment, etc., which was caused by my child's inapproriate behavior.

  • Admission Policy:

We affirm our commitment to accepting the full breadth and scope of Faith Mountain Academy’s program. We recognize that each child is a person with unique needs, and that our ministry may not be the best fit for every student. Faith Mountain Academy therefore reserves the right, after careful review of each application, to deny admission when a student’s educational needs are not best served by the program.
