• 2026 Academy Awards Report Form

  • Please use this form to submit Academy Awards. Please fill out one sheet PER RIDER. This form must be filled out in its entirety to be considered for awards. All results must be submitted to ASHBA by November 30. Medals will not be mailed out until after the 2027 ASHBA Youth Conference, but medals will be awarded onstage to those who attend the Youth Conference.
