PLEASE READ THIS SECTION PRIOR TO FILLING OUT THIS FORM.

YOU MUST AGREE TO THESE REQUIREMENTS TO BECOME A FOSTER WITH EAST TN PIT BULL RESCUE!

I can provide proof that I am at least 21 years old.

I understand I am fostering for ETPBR and have no right of ownership to my foster animal. The rights of ETPBR and for my foster animal are superior to mine, and an ETPBR representative may access my home and property to check on my foster animal as warranted

I agree to provide an ETPBR representative access to all parts of my home and property for a home inspection before my application to foster can be approved.

I agree to contact ETPBR with any and all questions or concerns about my foster animal or the foster care program.

I understand that I will be asked to transport my foster to adoption events, and to any vet visits that might be scheduled while they are in my care. All events and appointments will be scheduled in advance and with consideration of my schedule and availability.

I understand that I may be required to provide foster care for my foster for an indefinite period of time.

I understand that as long as I am fostering an animal to the satisfaction of ETPBR, I will be given the first right to adopt, when it is decided to place my foster animal up for adoption.

I agree to immediately return any foster animal in my care to ETPBR if requested by an authorized representative, at any time and for any reason. If ETPBR is forced to undertake legal action to enforce this provision of the agreement, I agree to indemnify ETPBR for all court costs, and attorneys fees connected with such an action.

If I plan to move during the time I am housing, a foster animal, I agreed to contact ETPBR prior to my move and provide my new contact information. I understand that ETPBR has a right to request the return of my foster animal based on my change of residence, and I agree to surrender my foster animal too ETPBR immediately upon request.

If at any point, I can no longer, or do not want to continue to provide care for my foster animal, I agreed to contact ETPBR to arrange for surrender and return of my foster animal.

I will take all necessary precautions to prevent my foster animal, if unaltered, from either impregnating another animal or becoming impregnated. Should this happen, I will notify ETPBR immediately.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless EAST TN PIT BULL RESCUE and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

I accept full responsibility for the dog(s) actions at all times, and release EAST TN PIT BULL RESCUE from any liabilities or damages that may be incurred because of fostering such dog(s).

I agree to have EAST TN PIT BULL RESCUE complete reference call checks and conduct a home visit inspection to be able to approve my foster application.

I agree that if I'm unable to foster the dog(s) anymore that I will return the dog(s) to EAST TN PIT BULL RESCUE and try to give EAST TN PIT BULL RESCUE a 2 week period to try and find another suitable foster for the dog(s).
