RELEASE & ASSUMPTION OF RISK & LIABILITY WAIVER

Assumption of Risk
I am the parent or legal guardian of the above-named minor and give permission for my child to participate in Ready Set Kick programs, activities, and events.

I understand that participation in athletic and physical activities involves inherent risks, including but not limited to injury, illness, falls, collisions, or other accidents. I knowingly and voluntarily assume all such risks on behalf of my child, whether known or unknown.

Release & Waiver of Liability
To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Ready Set Kick, its owners, officers, employees, coaches, volunteers, and affiliates from any and all claims, demands, damages, or causes of action arising out of or related to my child’s participation, including those arising from ordinary negligence.

This release does not apply to gross negligence or intentional misconduct.

Medical Authorization
I certify that my child is physically able to participate and that all known medical conditions, allergies, or limitations have been disclosed. I authorize Ready Set Kick to obtain emergency medical treatment for my child if necessary and agree to be financially responsible for any medical expenses incurred.

Insurance Acknowledgment
I understand that Ready Set Kick maintains liability insurance; however, I acknowledge that this waiver is intended to be a complete release of liability to the extent permitted by law.

Photo & Media Release
I grant permission for Ready Set Kick to use photographs or video recordings of my child for promotional or educational purposes unless I provide written notice stating otherwise.

Acknowledgment
I have read this Release & Liability Waiver, fully understand its terms, and sign it freely and voluntarily on behalf of my child.
