I attest that the horse is not registered with any other Passport Issuing Organisation (PIO) in Ireland or elsewhere in the European Union. I attest that the identifying veterinary surgeon checked for any previously implanted transponder before implanting a transponder supplied to the vet by SDI or another Irish PIO. I attest that another Irish PIO is not currently registering the horse.

I understand that to comply with the law, SDI must obtain, record, and store information about my name, postal address, email address, telephone number and EPRN. I understand that SDI must share this information with the Department of Agriculture, Food & the Marine (DAFM). This information must be held by SDI for a period of at least 35 years or at least 2 years from the date of communication of the equine’s death. For the sole purpose of payment for a horse registration, I consent to SDI obtaining my credit/debit card details for one-time processing of the fee. I understand that the credit/debit card details will not be recorded or stored by SDI. I understand that I have the right to access my information held by SDI, to have any inaccuracies corrected, and to have my information deleted if SDI is not required by the law to retain the information.

I understand that I can withdraw this consent at any time by emailing passport@sdi.horse and communicating my withdrawal of consent for SDI to hold any information except information that is required by law to be retained for the period mentioned above.
