  • If no, CHS can recommend a trainer for you if needed. CHS' dogs are in foster homes and raised with much love, so transitioning to a new home should be easy, but if necessary, would you agree to in-home-training? Our experience has been that the sooner you have a routine the easier it will be for you and your new dog. Please give your new dog at least two weeks to adjust.

  • Please share more about yourself.

  • Pet Health

  • Vaccinations, especially DHP and rabies are instrumental to your dog’s continued health.

  • Unfortunately, CHS often receives calls from family members, whose relatives have passed away and no one in the family wants the dog.

In the event, you or your family members can no longer care for the dog you are adopting today, who would be your backup? Who should CHS contact?
  • By signing this document, you agree to keep the dog you are adopting from CHS current on all vaccinations, heartworm and flea preventive and have regular medical checks (every six to 12 months in first year. I also agree not to dock tail, crop the ears or debark.

You also agree, if you decide at any time, you no longer want the puppy/dog, or for some reason you can no longer keep, you will call CHS (404-372-9545) and provide the organization with first right of refusal.
