1. Disclose my child’s registration information, including, but not limited to, name, date of birth, parent/guardian information, attendance records, and other relevant program data to the Government of Alberta as required for the Affordability Grant program under the Canada-Alberta Canada-Wide Early Learning and Child Care Agreement. I understand that the Government of Alberta will assign my child a Childcare Participant Number (CCPN) for the purposes of funding eligibility verification, grant administration, and regulatory compliance. All personal information is collected under section 33(c) of the Freedom of Information and Protection of Privacy Act (FOIP) for the purpose identified above.

5. Allow my child to be photographed or recorded in a classroom or field trip setting. Identifying images may be shared with registered Tree House families on REMIND (class information app). Non-identifying images ONLY may be used online for social media or advertising purposes. PLEASE NOTE: Tree House will NOT post identifying images of students without express permission from parent/guardian. See below.**

* Please be aware that we do allow parents and guardians to take images of their children during class time UNLESS OTHERWISE REQUESTED BY YOU. These images may include identifying images of other children in the class. Tree House Playschool accepts no responsibility or liability for third party use of any image or recording shared by third party on social media or elsewhere.
I am aware that $225 is required to complete registration. The $ 225 total consists of $150 registration fee per family (non-refundable) and $75 program fee per student (refundable until August 1, 2026). An additional $75 program fee per student is due January 1, 2027 (non-refundable).
I acknowledge that monthly fees are due by the first of each month of the program year and that late fees of $30/month will be added for fees outstanding for 30 days or greater.
I understand that 30 days written notice is required for withdrawal from the preschool program and where not provided, full monthly fees are due.
I understand that monthly fees will not be reduced due to partial absence, illness, program withdrawal or vacation.
