Clone of 2025 TAP Health Summer Academy Application
  • The 2026 TAP Health Summer Academy (THSA) is a healthcare careers exploration program that includes three cohorts—TAP HC, TAP MD, and TAP RN. You can apply to one or more using this single application. No matter which cohort you join, you’ll be a TAP Health student with access to events from all three.

To apply, a student will need to be a current high school sophomore or junior in the Greater Cincinnati, Northern Kentucky, or Southeast Indiana area.

    A few quick notes:

Note 1: TAP Health Summer Academy events are held in person at healthcare sites across the region. We are not able to provide transportation.

Note 2: All participants must follow current COVID‑19 and flu guidelines from state/local authorities and our host sites. These guidelines may change based on community health conditions.
As of January 15, 2026, COVID‑19 and flu vaccinations are not required to participate in THSA.

Note 3: All accepted students must provide documentation of a recent TB test and their MMR vaccination. These requirements are mandatory for participation.

Full application must be submitted on or before March 18, 2026. Please note this system will automatically generate a timestamp.

A few questions to get us started:

    • Personal Information
    •  -
    • Demographic Information
    • The next 3 questions in this section relate to demographics. The information helps us collect data for our evaluation records, and will not affect your selection into the program. The questions should be answered as to how you best identify yourself out of the choices provided.

    • Parent or Legal Guardian
    •  -
    • Education
    • This section gives us a little information about your school. In order to apply for this program, you should only do so with a school Educator (Guidance Counselor, Teacher or other administrator) aware of your application. This individual will be contacted by us if we have any questions or verification needs. We will also let them know the final status of your application. This person will be considered your School Contact.

    • Though it's not the most important part of your application, we are interested in learning about your standardized test scores.

If you've taken one or more - and are comfortable sharing for additional consideration - please select one of those tests in the choices below, and then upload the scores in the attachments section.

It is okay if you haven't taken a test or don't wish to share your current score; please instead select that option.

    • Extracurricular Activities
    • We would love to learn a little more about who you are! In the boxes below, please list a few activities you are or have been involved in. These can be unpaid, paid, school affiliated, non-school affiliated, etc. Please share years (or dates) of participation, as well as what the activity entailed. You do not have to complete every box, but we ask that you share at least two.

    • Personal Essay
    • We know essays can sometimes be tough, but please don't let that stop you from applying! We want our Selection Committee to get to know you in a more in-depth way. You don't have to be a great writer to submit an essay - just write with sincerity. The essay will be reviewed based on multiple factors, including content, passion, grammar, readability, flow and how well you generally answered the question. You will be given a choice in topics to write about.

There is nothing additional to upload - we have provided enough space within the application. We suggest having your essay ready (checked for grammar, word count, and for the content you want to say) in a program like Microsoft Word or an e-mail before you paste into the essay box.

    • Writing Supplement: Choose ONE out of the five (5) essay prompts below. This should also be no less than 250 words and no more than 800 words.

Be awesome. Be creative. Be yourself.

    • Financial Information & Waiver
    • If accepted, there is a $300 Participant Fee to be in one of our TAP Health Summer Academy programs. We will send payment details to accepted students at the time of their notification of acceptance.

If you are accepted into a program and unable to pay the cost, we offer financial waivers based on need. If you need this waiver, please have your parent or guardian sign the statement below.

    • Letter of Recommendation
    • Please ask a trusted adult to write a letter of recommendation for you. Give them plenty of time to submit the letter before the application deadline. It’s best to choose someone who knows you well—whether through school, work, sports, volunteering, or other activities. You may also ask someone who can speak to your character, strengths, or potential for a future in healthcare.

Letters of recommendation must be submitted separately and must come directly from the recommender via email to: taphealth@healthcollab.org

      The email should:

      • Include the student’s full name in the subject line
      • Include the recommender’s full name and organization, plus the student’s full name again in the email or attached letter
    • Submission
    • A few reminders before you submit:

      • Have you reviewed this form to check for spelling, grammar and content?
      • Is your most current school transcript attached appropriately?
      • Did you type your e-mail address correctly?
