Camp Waiver and Acknowledgement
I, the undersigned parent or legal guardian, give permission for my child listed above to participate in the Tennis Camp, which includes tennis instruction, drills, games, and recreational swimming activities (the “Activities”).
By registering my child for this St. Albans Camp program, I attest that my child is in good physical condition. My child has no physical impairment or condition that will prevent his/her participation in camp activities. I give my permission for the St. Albans staff to seek professional medical care if my child requires it.
I agree to abide by all program rules and policies and teach my child to do the same. I will assume all risk associated with my child’s participation in this program. I represent and affirm that my child is in proper physical condition to participate in the activities.
I understand that participation in tennis and swimming activities involves inherent risks, including but not limited to falls, collisions, heat-related illness, drowning, and other injuries. I knowingly and voluntarily assume all such risks, both known and unknown, on behalf of my child. To the fullest extent permitted by law, I hereby release, waive, and discharge the St. Albans Swim and Tennis Club, Phillip Hahn, Attack the Net LLC, its affiliate organizations and sponsors, its owners, directors, instructors, employees, volunteers, agents, board members, contractors, coaches and other volunteer persons used for the programs and facility providers from any and all claims, demands, actions, or causes of action arising out of or related to my child’s participation in the Activities, including those resulting from ordinary negligence.
Refund policy: Camp fees are non-refundable once registration is complete. Refunds will only be considered in the event of a documented medical condition that prevents the participant from attending.
Photo/Video Release: Additionally, I grant St. Albans permission to use my child’s name, image, voice and identity in any program for promotion of St. Albans including written materials, website materials on any and all St Albans publication across the entire worldwide web including but not limited to saintalbans.com and its sub domains, Facebook, Twitter, Instagram and any other unnamed form of social media, video, audio or other forms without prior consent or compensation of any kind.
Swim Waiver and Authorization
I, the parent/guardian of the registered athletes, agree that the participants will abide by the rules of St. Albans Swim Club. I recognize the possibility of physical injury or death associated with swimming and diving activities. I hereby release, discharge, and/or otherwise indemnify St. Albans Swim and Tennis Club, Phillip Hahn, Attack the Net LLC, its affiliate organizations and sponsors, their employees and associated personnel, including the owners of the facilities, board members, volunteers, agents, contractors, coaches and other volunteer persons used for the programs, against any and all claims, demands, actions, or causes of action arising out of or related to my child’s participation in the Activities, including those resulting from ordinary negligence.
Hold Harmless and Indemnification
I agree to indemnify and hold harmless the released parties from any loss, liability, damage, or cost (including reasonable attorneys’ fees) arising from my child’s participation in the Activities.
Governing Law
This agreement shall be governed by and interpreted according to the laws of Pennsylvania.
I have read and fully understand this Waiver and Release. I sign it freely and voluntarily on behalf of my child.
