  • Hold Harmless and Liability Waiver Agreement

    By signing below (or submitting this form), I acknowledge that I am voluntarily participating in activities organized by Music Changing Lives and its affiliates, partners, staff, and volunteers. I understand that participation may involve physical activity and potential risks, including injury or property damage.

    I hereby release, waive, and hold harmless Music Changing Lives, its officers, employees, partners, agents, and affiliates from any and all liability, claims, or demands arising out of or related to any loss, damage, or injury that may occur while participating in or traveling to and from any volunteer activity.

    I understand that I am responsible for my own safety and that this release applies to all acts of negligence, whether active or passive, on the part of Music Changing Lives. I certify that I am physically capable of participating and will follow all safety instructions provided.

    I have read and understand this agreement and sign it voluntarily. 

     

    Acuerdo de Exención de Responsabilidad

    Al firmar a continuación (o enviar este formulario), reconozco que participo voluntariamente en las actividades organizadas por Music Changing Lives y sus afiliados, socios, personal y voluntarios. Entiendo que la participación puede implicar actividad física y posibles riesgos, incluyendo lesiones o daños a la propiedad.

    Por la presente, libero, eximo y eximo de responsabilidad a Music Changing Lives, sus directivos, empleados, socios, agentes y afiliados de toda responsabilidad, reclamación o demanda derivada de o relacionada con cualquier pérdida, daño o lesión que pueda ocurrir durante la participación o el desplazamiento hacia o desde cualquier actividad de voluntariado.

    Entiendo que soy responsable de mi propia seguridad y que esta exención se aplica a cualquier acto de negligencia, ya sea activa o pasiva, por parte de Music Changing Lives. Certifico que soy físicamente capaz de participar y que seguiré todas las instrucciones de seguridad proporcionadas.
