This form helps us collect important information to ensure proper placement and overall safety. Once submitted, Sasha will reach out to confirm details, availability, and next steps for your appointment.

1. Age Requirement – All clients must be 18 years or older and present a valid photo ID at the time of the appointment. 2. Minors – Clients under 18 must be accompanied by a parent or legal guardian, who must be present with their own valid photo ID to sign for the procedure. 3. Late Arrivals – To respect everyone’s time, please arrive 10 minutes early to your appointment. A 10-minute grace period is allowed. If you arrive more than 10 minutes late, your appointment will need to be rescheduled, and a deposit will be required to secure the new booking. 4. Rescheduling – I understand that life happens. If you need to reschedule, please do so at least 24 hours before your appointment. A second reschedule will require a deposit to hold your spot. 5. No-Shows – Failure to attend your appointment without notice will require a deposit to book again in the future.
